Eligibility Guidelines

staff_4.jpgThe Open Bible Medical Clinic serves patients in the El Paso County community with primary medical care for acute and chronic illness as well as provide in house limited mental health counseling. All of our dedicated and professional staff are volunteers.

Employed patients must qualify at or below 200% of the Federal Governments poverty level guidelines, have no health insurance coverage or be eligible for state or federal coverage, (Medicaid or Medicare), nor be a member of Peak Vista Community Health Center (PVCHC), S.E.T., or Mission Medical.

Eligibility and Documentation Requirements

Those Who Do Not Qualify:
Patients with prescription insurance, Medicare or Medicaid patients, members of a FQCHC, children under 18 and veterans with benefits DO NOT qualify for medication assistance from Open Bible Medical Clinic or TLC Pharmacy.

1.      Photo Identification
2.      Proof of residency in El Paso or Teller Counties:
- Lease and most recent Utility Bill
3.     Proof of Income:
        Mandatory Items (if applicable)
        - Income Tax Forms are required (if you’ve filed) / previous year W-2 (if you’ve worked & not yet filed)
       
        - Disability letter from Social Security Administration (if you are receiving disability)
        - Food stamp letter showing the amount you receive monthly (if you are receiving this assistance)
(If you have not filed Income Tax documents in recent years or
don’t have a W-2), Please provide the following:
        - Pay Stubs from previous 2 months
        - Bank Statement for automatic deposits for 2 previous months
        - Unemployment Compensation Voucher

We predominantly serve adults as PVCHC, El Paso County Health Department, and the state of Colorado have programs for children and pregnant women (CHP+). We do not provide physicals,  ”well” visits or any OB-GYN type of appointments.

 

We do provide medications for our patients through the TLC Pharmacy if available at no cost, however we do not fill prescriptions from any other provider. For outside prescriptions we can provide you with a discount card that can be used at any of the major pharmacies in town and will save you anywhere between 20-60 percent of the retail cost.

All of our services are free however some of our referrals may not be. In most cases basic Lab work and x-rays will be covered through partnership arrangements. Each outside referral is done on a case-by-case basis, please check with the agency you are referred to for payment guidelines.
